Admissions/Applications
There are 2 ways to get the application forms:
- You can complete the main application online. (Recommended)
- You can request that hard copies of all the forms be sent to you via regular mail, however, it is strongly recommended that applicants apply online using the above link.
For Item 10, Part A, of the application form, you should enter the appropriate department code:
- 3536 for the M.S. degree Rehabilitation.
- 5036 for the M.S.P.H. degree Public Health in Community Health
- 6236 for the M.S. and PhD degrees in Community Health
If submitting your application by mail, please note that, although the application form indicates that it should be sent to the Office of Admissions and Records, it should be sent directly to the Department of Kinesiology and Community Health at the address below. Also note that, if you submit your application online (using option 1 above) you do not need to also send a completed application form by mail.

Official Transcripts
You must request official transcripts of all of your undergraduate and graduate work to be sent by the institution directly to the Graduate Program Coordinator, Department of Kinesiology and Community Health. Transcripts must show courses completed, grades, degree completed, and date of graduation. If you are in process of completing your degree, and expect to do so before you would enter the University of Illinois, have the current transcript sent now. After your degree is completed you must arrange for another transcript to be sent. If these records are in a language other than English, they must be accompanied by an official English translation.
Results of the Graduate Records Examination
The Graduate Record Examination (GRE) is administered by the Educational Testing Service. The results must arrive at the Department by the deadline mentioned above. Therefore, make sure that you make arrangements and take the test on time. Note that the Institution Code that you must enter in the GRE application for the University of Illinois at Urbana-Champaign is R1836 and the Department Code is 0616 (listed as "Public Health" in the GRE booklet).
Personal Statement
This statement is a very important part of your application. Please include comments on why you are interested in the Community Health program, what is your current understanding of the field, what education and skills you expect to acquire through the degree, and how you expect to use your degree after graduation. Briefly describe any previous health or rehabilitation related experience including volunteer time, paid work, internships, coursework, and pertinent training. The Personal Statement is particularly important for applicants who completed their last degree five or more years ago. Applicants with professional work experience are encouraged to explain how they think their work experience has prepared them for graduate study at the University of Illinois. Please type your statement of interest on this form. 
Letters of Recommendation
Request three letters of recommendation (preferably from those who are acquainted with your educational background and academic abilities) be sent to the Department of Kinesiology and Community Health at the address below. The letters of recommendation must be typed on this form (remember that you need 3 copies).
